FYI Safety Score

1.2 Poor

The Preserve at Woodland Hills has accumulated 41 state citations (28 serious, 13 minor) and 27 verified complaints since 2021 across 52 inspections. A 2024 investigation substantiated that 2 staff members photographed a resident while on the toilet and shared the image among personal contacts, resulting in their suspension and termination. A separate November 2024 complaint investigation found that 1 caregiver was responsible for approximately 25 residents in the dining room and left the group unattended for roughly 20 minutes while assisting another resident; a kitchen staff member told inspectors this occurred about half the time and described the facility as very short-staffed. Also in 2024, inspectors cited the facility for incomplete staff training after records showed staff had not fulfilled required initial and annual continuation training hours. In December 2024, an inspector found that a resident's evening medications were not administered on 12/18/2024 and that documentation of the refusal was entered the following day rather than in real time. The facility was last inspected in March 2026.

Why we like it

The Preserve at Woodland Hills is a 60-bed assisted living and memory care community located on Fallbrook Avenue near Ventura Boulevard in Woodland Hills. The facility offers both general assisted living and a dedicated memory care neighborhood with dementia-trained caregivers, secured environments, and therapeutic programming.

Amenities include restaurant-style dining, a full-service salon, scheduled transportation, and daily enrichment programs. The website notes the community is under new management.

  • Dedicated, secured memory care neighborhood

The memory care program is separate from general assisted living, with its own secured environment, specialized staff training, structured therapeutic activities, and family education programs. This purpose-built approach is what families dealing with Alzheimer's or dementia should look for.

  • Mid-size community that balances scale and intimacy

At 60 beds, The Preserve is large enough to offer real amenities — dining room, salon, activity programs — but small enough that residents aren't lost in a crowd. It's a middle ground between a 6-bed care home and a 200-bed campus.

  • Restaurant-style dining and lifestyle amenities

Chef-prepared meals, full-service salon, scheduled transportation, and daily enrichment programs are standard. These quality-of-life details matter at every meal and every day.

  • Below-average safety record

The FYI Safety Score™ is 1.1 out of 10 (Poor), based on state inspection data. This reflects a significantly higher-than-typical number of citations and substantiated complaints. Families should ask management directly about their inspection history and demand specifics about corrective actions.

  • Under new management — the track record is resetting

The website explicitly states 'Now Under New Management.' While this could signal positive changes ahead, it also means the team has limited history to evaluate. Ask what has changed, why the transition happened, and what improvements are underway.

  • Only 4 years of licensed operation

Licensed since 2021, this is a young community with a short track record. Combined with a management change, there's limited historical data to assess long-term quality. Ask about staffing stability and resident retention.
