An October 2025 complaint alleging unexplained injuries was determined unfounded when investigators confirmed the individual did not reside at the licensed facility but at an adjacent skilled nursing facility under separate regulatory oversight. Annual inspections in 2022, 2023, 2024, 2025, and 2026 resulted in no citations.
