In December 2025, inspectors found the home was keeping a bedridden resident without the required fire safety clearance, and daily fines of $100 were assessed while the situation continued. In January 2026, the state called the licensee in for a compliance meeting to address the ongoing pattern, which included incomplete medication records, missing resident care plans, and the licensee's business entity having been suspended by the state for unpaid taxes. By April 2026, inspectors returned and found that medication record-keeping requirements were still not being met.
