Also in 2024, staff were cited twice for speaking Tagalog during direct hands-on care with English-speaking residents who had dementia diagnoses; a civil penalty was issued for the second violation. A 2023 complaint confirmed staff failed to notify a resident's physician of a change in condition. The most recent complaint, investigated in August 2025, involved a resident who developed pressure injuries following a January 2025 hospital discharge. Annual walkthroughs in 2022, 2023, 2024, and 2025 found no violations during routine inspections.
