Inspectors making monitoring visits in October 2023 found medications and a wound cleanser left unlocked in a resident's room and a smoke detector removed from a ceiling. In November 2023, inspectors confirmed the home had been without telephone service for about five days. In April 2024, inspectors found the home's freezers and pantry were only half-stocked. Investigators also looked into other complaints, including an allegation that a staff member punched a resident, but couldn't confirm it happened.
