In 2022, inspectors visiting after a complaint found the entrance door alarm had been disabled while a resident with wandering and sundowning behavior was living there. That same visit found medications unlocked in the refrigerator, knives and cleaning supplies in unsecured kitchen drawers, and cleaning products in an unlocked utility room. Also in 2022, after a resident fell unwitnessed and was hospitalized, staff could not find that resident's file when inspectors asked for it. In 2021, inspectors issued a serious citation because the home had added three rooms inside the staff bedroom without approval. In late 2024, the home was fined $500 for failing to complete the required care assessment for a bedridden resident with advanced dementia, contractures, and a fractured pelvis; it was also cited that year for not responding for over a month to a family's repeated requests for a resident's records. Investigators looked into a number of other complaints over the years, and some were not confirmed.
