In August 2024, inspectors documented approximately 14 instances where staff took over 30 minutes to respond to resident pendant calls, with similar delays in oral care and toileting assistance. A serious citation with civil penalty was issued in August 2024 after an employee was found working without the required California background clearance. A September 2025 complaint substantiated that only 1 functioning washing machine was available for all residents, with the memory care unit washer confirmed out of service.
