A staff member was found to have made over $3,500 in unauthorized purchases on a resident's bank card, and the home was ordered to reimburse the resident. In 2022, inspectors confirmed a bedbug infestation and that staff waited five days to seek medical care for a resident who had bug bites, without telling the family. Pest issues returned: inspectors confirmed in 2024 that residents were being bitten again. The home was cited in 2023, 2024, and 2025 for not having enough staff, including no awake overnight staff; a September 2025 investigation confirmed no one was scheduled between 10 PM and 6 AM. In November 2024, a resident left the home unsupervised at night and was found disoriented in San Jose at 3 AM, needing hospital care; the family filed the missing persons report, not the home. A month later, a resident went missing before dinner, and the state fined the home $1,000 for a repeat offense. Inspectors also found a resident living in a converted room that had not been fire-cleared, and observed exposed wiring in another room. Several other complaints over the years were investigated and not supported by the evidence.
