A serious state citation from October 2024 found that a private caregiver working at the facility lacked the required criminal record clearance, resulting in a $500 civil penalty. In November 2025, inspectors documented 2 separate incidents in which an outside intruder twice entered the building through a loading dock, reached a resident's floor, and physically confronted a resident before being removed by security and reported to police. Annual inspections in 2024 and 2025 found safety systems, staffing, and supplies to be in order with no immediate health or safety concerns noted.
