A separate verified complaint found a resident with multiple bruises on both forearms, the right hand, and a skin tear on the bicep; other residents reported that staff pushed the resident against the wall or bed rail during repositioning. A November 2024 unannounced case management visit cited oxygen stored without required door signs, broken window screens, medication left unsecured on the kitchen counter, a new staff member working without completed training, and surveillance equipment recording audio in resident bedrooms in violation of resident rights. A noncompliance conference in 2022 addressed chronic severe violations under a prior administrator; the facility changed ownership in late 2021 and has operated under new management since.
