In December 2022, inspectors confirmed a complaint that a staff member pulled a pin out of a drilled hole in the front door frame, which let the door stay closed even when it was unlocked. A witness who toured the home in November had seen this. The inspector saw the hole and also saw locks at the bottom of the back sliding glass door that could keep it from opening. During that visit, the inspector also saw a chair, clothing and shoes blocking the sliding glass door in a resident's room, and staff helped move them into the closet. In July 2023, inspectors confirmed that staff did not respond when a resident pushed their call button. Staff said the resident kept pushing it in the middle of the night to ask what time it was, so they removed the call button. Other complaints the state looked into in 2023 and 2025 were not supported by the evidence.
