A May 2025 incident report in which a resident described being dragged to the bathroom was reviewed with no deficiencies issued; the resident clarified they meant being pushed in their wheelchair too quickly, and no injuries were noted. Annual inspections in 2024 and 2025 identified no health or safety concerns.
