The most significant verified complaint, investigated in July 2025, found that a resident missed multiple medication doses on at least 2 occasions because staff allowed the resident to sleep through scheduled administration, and a review of medication records from October 2023 through June 2025 revealed multiple instances of medications not marked as administered. Staff acknowledged forgetting to document administrations, and the facility held a medication documentation in-service training in May 2025 in response. A May 2025 informal conference with state regional managers resulted in the facility agreeing to update its 911 response policy and plan of operation. The remaining 14 complaint investigations, covering allegations including falls, pressure injuries, hygiene care, infection control, double billing, and failure to notify authorized persons, were either unsubstantiated or inconclusive. Case management visits following resident falls in 2025 found no health and safety concerns and resulted in no citations.
