The most significant findings came from a 2021-2022 state audit of how the home handled residents' personal money: auditors found that transactions involving residents' funds were not properly recorded, and some documents appeared to have been prepared after the fact rather than at the time of the transactions. Inspectors cited the home for these issues, and by May 2022 it had been placed on a three-year compliance plan. During a 2022 inspection, inspectors also found that a resident had a full bed rail in place without a required prescription, and that the home was short on perishable food that day. In 2024, inspectors cited the home for failing to report a resident's fall as required. The state investigated a number of other complaints over the years, including allegations involving resident care and falls, and did not find enough evidence to confirm any of them.
