Since 2024, state inspectors have cited Dignity Senior Care Inc. 33 times (22 serious, 11 minor) and verified 9 complaints across 31 inspections. A recurring finding across multiple visits: the home's fire clearance covers only residents who can move around on their own, but inspectors found residents living there who could not, both during a complaint visit in May 2025 and again at an annual inspection in September 2025. During that September 2025 annual visit, inspectors also found cleaning chemicals left unlocked under the kitchen sink and accessible to residents, a carbon monoxide detector that was not working, and not enough food on hand. A complaint from the same period was also substantiated: inspectors reviewed reports and photos from another agency that had visited in July 2025, which found centrally stored medications in the refrigerator and kitchen cabinet left unlocked and unsecured, cleaning supplies and hazardous chemicals unsecured in a kitchen cabinet, unlabeled food in both refrigerators, and vegetables stored in a cabinet next to hand sanitizer without proper sealing. On that same September visit, a staff member was found working without a required background check on file, and the home could not produce resident or staff records for review. Inspectors returned in October 2025 and cited the home again for missing resident and staff paperwork and a toilet with a missing seat. State inspectors looked into many other complaints over this period, including concerns about staff neglect, medication management, and a resident being abandoned at the hospital, and most didn't hold up when investigators looked into them.
