Following the incident, the staff member was removed from medication duties and the facility updated its administration process to add resident photos, names, and room numbers on medication cups. A separate complaint from 2025 was also substantiated, confirmed by a follow-up inspection in October 2025. In 2023, inspectors observed rodent droppings throughout the kitchen, storage room, and near the reception desk, and found that the facility's pest management contractor was not treating for rodents at that time. A citation was also issued in late 2025 for failure to ensure appropriate communications with a resident's responsible party.
