In October 2025, inspectors confirmed the home refused to take a resident back after a hospital stay without providing an eviction notice or doing a new assessment. Through late 2025, inspectors found the home still working through multiple requirements that had already been flagged for correction, including supervision, administrator qualifications, and staff training, with a new acting administrator brought in to help. Inspectors looked into other complaints over the years and did not find enough evidence to confirm them.
