Additional case management inspections in September 2025 cited deficiencies for a 4-day missed medication dose and a resident fall in which a staff member placed the resident's walker out of reach and left them on the floor without assistance. In March 2025, an outside agency caregiver pulled a Memory Care resident by the wrist into their apartment, causing the resident to fall and strike their head. The most recent annual inspection in February 2026 found no deficiencies.
