A July 2025 complaint on the same issue resulted in a serious citation. In December 2023, inspectors substantiated a complaint that a resident who had fallen waited 94 minutes before receiving a staff response. In June 2025, a substantiated complaint documented a pipe burst that left residents without running water for several hours, requiring bottled water and portable toilets until service was restored the same evening. A December 2025 annual inspection cited food safety violations, including uncovered prepared meats and other perishables in the kitchen. In late 2023, investigators also noted the facility was operating without a certified administrator on record.
