Since 2021, state inspectors have cited Brookdale Chanate 43 times (24 serious, 19 minor) and confirmed 22 complaints across 49 visits. The most serious incident on record happened in July 2022, when a memory care resident went missing from the home; police were called, and the resident was found about 2.5 miles away by a community member, taken to the hospital, and diagnosed with a stress fracture. The home was fined $500. That same year, inspectors confirmed a complaint that a staff member yelled at a resident in a sharp, loud way and failed to treat them with dignity. Response to call buttons became a recurring problem: in October 2023, when the call button system went down, staff were supposed to check on residents every two hours but couldn't always meet that schedule, and at least one resident's needs went unmet; the following month, records showed residents' call pendants had gone unanswered on multiple occasions, and the home was fined $250 for repeating the same pattern. Inspectors also confirmed in late 2023 that not all staff knew how to get supplies to residents, and that the home hadn't properly notified a resident's representative when a resident died. In late 2024, inspectors found that a resident had been given the wrong medication dose for about three weeks, and that the home had never reported the error to the state. Many other complaints were investigated over the years, and some were not supported by the evidence.
