Inspectors found the home hadn't updated a resident's medical assessment, and that resident's physician's report hadn't been updated since August 2020. In October 2021, the state found that a room approved for bedridden residents didn't meet Fire Code requirements for that use. The licensing analyst told the administrator not to accept bedridden residents in that room until further guidance. In January 2026, a state analyst hand-delivered an immediate exclusion letter about a staff member to the administrator, who said that person no longer worked at the home and would be removed from its roster. No citations were issued that day. An April 2023 complaint alleged that staff were slow to get medical help for a resident and didn't keep the family informed of changes in the resident's health. Inspectors found the staff had applied an ice pack and called 911 once the resident said they were in pain. They also found a history of texts and progress notes updating the resident's family. Only the November 2021 complaint was confirmed.
