Since 2025, state inspectors have cited Sacramento Senior Living III 22 times (7 serious, 15 minor) and confirmed 11 complaints across 12 inspections. The most serious findings involve residents leaving the home without staff knowing on two separate occasions. In August 2025, inspectors found overnight staff were asleep rather than awake supervising residents, and cited the home for not meeting residents' care and supervision needs. Then in October 2025, a resident got out by turning off the alarm, and the state fined the home $500 for that incident. Inspectors also confirmed a rodent and insect problem in fall 2025: residents told inspectors there were mice, cockroaches, and flies in the home, and inspectors found a hole in the exterior wall that pests were getting through; the home had brought a cat in to deal with the mice. The home was also cited for failing to file a required incident report on time after a resident was sent to the hospital. A resident died at the hospital in December 2025; state records show the cause was attributed to multiple underlying conditions. In March 2026, state licensing staff met with the home's owner to address concerns about staffing levels, medication handling, care documentation, and reporting.
