A 2023 complaint was substantiated after the facility waited 16 hours to notify a family member of a fall, contacting them only once the resident reported pain. A November 2024 annual inspection found that the executive director's administrator certification had been expired for more than 7 months with renewal still pending. Earlier citations from the 2022 annual inspection included hot water temperatures above the required range and outdated physician records for 4 of 5 resident files reviewed. The most recent inspection in October 2025 observed the facility to be clean, odor-free, and in good repair.
