Across 49 inspections, state inspectors have cited Legacy Oaks of Sacramento 41 times (27 serious, 14 minor) and confirmed 23 complaints. In January 2025, a resident whose care plan required checks every 2 hours was found unresponsive by housekeeping in the morning; the last documented contact was around 2 a.m., with no record of any check in between. The state opened an investigation into the death, but its outcome isn't included in the reports available. In February 2026, inspectors confirmed a serious finding that the home was not regularly monitoring residents for changes in their condition. Inspectors also confirmed in a separate investigation that staff were not following infection control guidelines. During a visit in August 2025, inspectors found the memory care rear exit was unalarmed and opened freely to the back parking lot, a laundry room was unlocked with detergent within reach of residents, and the medication room door was open with the keys in the lock. A review of call logs from May 2025 showed staff took more than an hour to respond to a resident's emergency call button on multiple occasions, with one call going unanswered for 2 hours and 26 minutes. An inspection in December 2025 found 2 of 3 staff files reviewed were missing required annual training. In October 2025, inspectors investigated a complaint that the home's central air conditioning had failed to keep rooms cool on a day when outdoor temperatures reached 105 degrees, and staff acknowledged the AC had been an ongoing problem. Many other complaints were investigated and not confirmed.
