Since 2021, state inspectors have cited Greenhaven Estates 72 times (43 serious, 29 minor) and confirmed 32 complaints across 91 visits. In June 2021, a resident was admitted and removed by the home that same day. The home said the resident hadn't brought a 30-day supply of medications, but inspectors found no such requirement anywhere in the home's own agreements or operating plan, and the home could not say where the resident was for the next two days while their belongings remained on-site; inspectors confirmed the home had no documented basis for the removal. That fall, inspectors also confirmed the home was not following required mask rules. In 2022, inspectors confirmed three residents developed pressure sores while in the home's care, with one progressing to stage 3. In 2024, inspectors found that a staff member had only two hours of dementia care training that year, well short of the required eight, and had no first aid certificate on file; they also confirmed the home's air conditioner had been out of service from May 10 to May 29, with the home waiting until May 22 to call a repair contractor, and that a resident was hurt as a result. In November 2025, inspectors confirmed that staff were logging medications hours after giving them rather than at the time of administration; a separate investigation the same day found the home's training shortfalls were recurring, and the home was fined. Many other complaints were looked into over the years, including allegations of sexual abuse, theft, and rough handling of residents, and investigators did not find sufficient evidence to support them.
