In October 2022, a staff member locked their bedroom door and fell asleep at midnight, leaving residents without care or supervision; a resident left the home without permission on two separate nights that week, and the state fined the home $500. In 2023, inspectors found that one resident's prescription bottles had been placed in a different resident's medication box, and a separate resident's medication record listed the wrong dose for a psychiatric drug. By late 2024, inspectors found a caregiver who was signing off on giving a resident their inhaler and diabetes monitor but told inspectors they had never seen the inhaler and did not know where the monitor was. That same year, inspectors found an open pocketknife with the blade extended on a small table next to a resident's bed, alongside unsecured medications; because the home had been cited for unsecured medications just weeks before, the state fined the home an additional $250. In November 2024, the state ordered one staff member immediately excluded from all licensed care facilities, barring them from working in, living in, or having any contact with residents. On a separate visit, inspectors found feces and toilet paper dried and caked onto the outdoor patio, which staff said had been there for at least two weeks, and cited the home for using unpermitted backyard sheds as living quarters. Across multiple visits in 2024 and 2025, inspectors also found caregivers working without the required state background clearance and with incomplete personnel records on file.
