In April 2025, inspectors asked twice to review documents from a resident's file, and both times the documents were locked in the executive director's cabinet while the director was away; records must be available to inspectors on the spot. A follow-up visit in May 2025 found the home was missing a resident's medication records and had no written reports on file for that resident's 2 hospital stays in December 2024, which must be reported to the state within a week. Inspectors also looked into other complaints during this period and did not confirm any of them.
