Inspectors also documented recurring medication errors in 2025: a case management visit in August 2025 cited deficiencies related to 2 separate incident reports involving medication errors, and a December 2025 follow-up visit cited deficiencies tied to 2 additional errors, including eye drops administered to the wrong resident and a dosage error, with staff training provided after each. The facility saw a change in executive director in December 2024. The most recent annual inspection in January 2026 found the facility clean, safe, and in good condition with no immediate health or safety violations observed.
