The most significant finding came from a January 2023 complaint investigation: a staff member repeatedly wrapped a resident's legs too tightly during care, causing pain, and ignored the resident's requests to stop. The complaint was verified and resulted in a serious citation. A September 2023 annual inspection yielded 1 minor citation; a separate complaint investigated during that same period was found unsubstantiated. In November 2025, 16 residents reported missing personal belongings from their rooms during the period from August through November 2025. The facility responded by implementing visitor badge requirements and additional security cameras. As of the January 2, 2026 follow-up visit, the investigation remained open and no citations had been issued. Annual inspections in 2024 and 2025 found no deficiencies. The facility recently transitioned to new executive leadership and is undergoing construction to renovate common areas and add a bistro, with completion expected August 2026.
