State inspectors have cited Roseleaf Oroville 62 times (38 serious, 24 minor) and confirmed 52 complaints across 116 visits. Among the confirmed findings: in 2025, inspectors found that a staff member was giving residents medications without having completed the required training. In 2023, inspectors confirmed that one resident's call light cord was missing, and the resident could only reach the button if their bed was pushed directly against the wall. In 2024, room temperatures in parts of the home dropped to 62 degrees, below the required minimum of 68, and inspectors also cited the home that year for dirty floors, food on the dining room floor, a cluttered resident room, and running out of laundry detergent. The home has been cited on multiple occasions for maintenance problems, including cracked and missing flooring in the laundry room, broken light fixtures, and missing cabinet doors and drawers. In late 2025, inspectors found the home had unpaid utility bills and vendor invoices going back more than a year, including a disconnection notice from PG&E and overdue water and sewer bills. Many other complaints were investigated over the years and were not confirmed by the evidence.
