Among the most serious: in October 2022, inspectors found the home without emergency oxygen, with a fire alarm in disrepair, missing emergency lights, obstructed walkways, a hole in a bedroom wall, and broken and rusted shower tiles. Inspectors also confirmed that medications were not stored properly and medication records were not being kept. Inspectors also observed dead roaches and rodent droppings that same visit. In May 2023, rodent droppings were confirmed again, and the home was fined because it was a repeat issue. In April 2023, inspectors confirmed that a staff member yelled at residents and handled residents and their belongings roughly; the staff member's required resident-rights training had also lapsed. That same month, the home was cited for illegally raising a resident's rent without proper written notice and for attempting to evict a resident by text message instead of the required written process. In September 2023, inspectors confirmed residents were given verbal eviction notices before the required written ones. In May 2024, inspectors confirmed that a resident was left in a soiled diaper after a caregiver request to change them was ignored, and another resident was found unkempt with long fingernails and toenails. Many other complaints were looked into over this period and not confirmed.
