Since 2021, state inspectors have cited The Artesian of Ojai 70 times (38 serious, 32 minor) and confirmed 46 complaints across 54 visits. In July 2021, a resident fell three times, on July 22, July 26, and July 27, and the home never reported those falls to the state; inspectors also found the resident's care plan had not been updated since the day they moved in. Over late summer and fall of 2021, inspectors confirmed repeated staffing shortages across dozens of shifts: the home's Topa Topa building routinely ran with fewer aides than required, and on multiple shifts no medication technician was available to handle medications for all three buildings. Inspectors also confirmed that after a resident came home from the emergency room with a blood clot, the prescribed blood thinner Xarelto was not given at two consecutive doses; in a separate case, a pain medication (oxycodone) was given twice within 18 minutes, well before the prescribed four-hour interval. In 2021, inspectors also confirmed other medications were not given to residents as prescribed. In 2022, inspectors confirmed that after a resident died, the home did not return the family's money within the 15-day window its own contract required. In 2023, inspectors confirmed the home refused to share a resident's records and incident reports with their family; the care director admitted she had denied records while not knowing what families were entitled to see. In 2023 and again in 2024, inspectors confirmed the home was not keeping required records of residents' personal belongings. In 2025, inspectors confirmed that emergency pull cords in resident bathrooms were wrapped around grab bars and could not be reached from the toilet; staff said they had seen the cords tied up but left them that way. State investigators looked into many other complaints over the years and in some cases, including concerns about pest control and hot water access, did not find them supported by the evidence.
