The most serious finding involved a substantiated complaint from May 2025 confirming the facility was in financial distress: rent payments had gone unpaid back to January 2025, and an unlawful detainer was served by the landlord in May 2025. The licensee also failed to report either the financial issues or the eviction filing to the state, as required, resulting in a second substantiated deficiency. A separate April 2025 visit found that residents admitted before the change of ownership lacked updated admission agreements, and 2 residents admitted after the new license was issued had no signed admission agreements on file at all. State regulators responded with a non-compliance conference in July 2025, followed by a series of unannounced case management health checks running from March through April 2026. Those recent visits found residents in clean clothes, bedrooms free of hazards, bathrooms clean and equipped with grab bars, and food and utilities in order, with the facility carrying 3 to 5 residents at the time of each check.
