A separate 2022 investigation confirmed that medication was not dispensed to residents and that call buttons were not answered in a timely manner. In September 2024, a fall from the prior month had not been reported to the state. The March 2026 annual inspection noted water temperatures above safe limits in 1 bathroom and incomplete staff and resident files. The facility changed administrators between 2022 and 2024.
