In May 2025, a citation was issued after a staff member administered another resident's medications to a different resident; that resident experienced a change in condition, was evaluated at the hospital, and returned the same day. In February 2025, a citation was issued after staff providing contracted one-to-one care twice left to assist other caregivers, though the resident remained within eyesight and under supervision. Annual inspections in 2024 and 2025 found the facility clean, well-maintained, and in good repair with no deficiencies.
