Since 2021, state inspectors have cited Whitten Heights 54 times (24 serious, 30 minor) and confirmed 46 complaints across 108 visits. The most serious: in 2021, a resident repeatedly attacked other residents and a staff member over several months, causing injuries, and inspectors found the home had not updated that resident's care appraisal for two years despite a documented worsening in behavior. In 2025, inspectors confirmed a resident sustained multiple falls while the home had never completed a required care appraisal for that resident. Physical conditions have drawn repeated citations as well: in 2021, inspectors found extensive carpet staining and possible mold on all three floors and fined the home $250 for a repeat issue; in 2024, broken flooring in the memory care areas was confirmed as a trip hazard, along with an exposed dryer flame and exposed washing machine cables; and in 2025, inspectors again confirmed the chairs and recliners in the memory care were heavily stained, unsanitary, and torn. Inspectors also confirmed in 2024 that a resident fell after a dog ran through the hallway and that residents were smoking inside their rooms. Many other complaints were looked into over the years and were not confirmed.
