During one investigation, records showed staff were giving a resident far more alcohol than the person authorized to make decisions for him had approved: 2 to 3 servings in the evening, each 2 to 4 five-ounce cups, against written permission for only one small 2-ounce bottle at night. Records also showed there were evenings when he received alcohol as late as 1:00 am, 1:20 am, and 1:30 am. On the night of January 14, 2023, he was found unresponsive on his floor and taken to the hospital, where doctors diagnosed alcohol intoxication with a blood-alcohol level of 244. During another investigation, records showed a resident whose care plan required two-person assistance for transfers had been moved by one person; a family member reported witnessing this multiple times, and 6 incident reports documented her falling over several months. On July 26, 2023, she fell, could not get up, and was taken to the hospital, where she was found to have a fracture near her knee. In December 2024, inspectors found that staff without nursing credentials were giving insulin injections to residents, a task that requires a licensed nurse. That same visit, inspectors found dead roaches in two residents' apartments and in a kitchen cabinet, and the home was fined $250 because it had been cited for the same problem before. Inspectors cited the home three times for kitchen staff working without required hairnets, with a $250 fine on the third occasion, and confirmed a complaint that the home repeatedly failed to respond to a family member's months-long effort to resolve a billing refund.
