In 2022, inspectors confirmed that a resident's door malfunctioned repeatedly over about 2 weeks, needing new batteries and replacement parts, while the resident had to keep asking staff to open it. That same year, inspectors reviewed hospice records showing 3 residents on fentanyl pain patches were each found by nurses with their patches missing; nurses placed new patches and reported the issue to the home each time. In 2025, inspectors found that a resident's care plan and fee increase were changed after a fall without properly notifying or meeting with the family first. An inspector's 2023 medication audit found multiple prescriptions with incorrect counts, including 1 that was short by 125 tablets. Inspectors confirmed the home did not provide a resident's family with a complete copy of their loved one's records when requested. Inspection records from 2024 also document a resident trapped in their bathroom, unable to get help; the resident bruised their hand, arm, and legs from banging on the door before a visitor found them and staff called 9-1-1. By late 2025, inspectors confirmed the home had no training records for staff who handled medications, with planning that had begun under a prior administrator left unfinished when that person departed. Many other complaints were looked into over the years, including concerns about staffing levels and food service, and not all were confirmed.
