Since 2022, state inspectors have cited Sunnycrest Senior Living 39 times (21 serious, 18 minor) and confirmed 37 complaints across 76 visits. Among the more serious findings: a resident was sent to the hospital and needed four stitches after staff caused a skin tear by pulling her from her wheelchair without lifting her foot clear of the bed, and a resident and a witness told inspectors that the care director had said the home didn't always have enough staff for the two-person lifts the resident required; an earlier December 2021 incident had already left her with a skin tear on her forearm, and a month after the hospital visit, she got another skin tear on her elbow while in the bathroom with a caregiver. Inspectors confirmed medication problems in 2022 and in 2025 cited the home for medication record discrepancies. In 2022, a medication technician entered a resident's room without the cart and laptop needed to verify the right drugs, nearly giving the wrong medication, and inspectors confirmed staff weren't properly trained; in 2025, medication records showed conflicting entries and crossed-out errors related to a complaint that a resident received another resident's opioid painkiller instead of her own. Also in 2022, inspectors confirmed roaches and ants in resident rooms, front and back entry doors left unlocked late at night, no working telephone on the premises, residents waiting up to an hour for staff to answer call buttons, and the home failing to provide a resident's records to her authorized contact. Many other complaints were looked into over the years and were not confirmed by the evidence.
