A 2023 complaint was substantiated after a state inspector found a non-staff individual sleeping on the living room couch and determined a staff member listed in facility records lacked the required background clearance; a $500 civil penalty was issued. That background clearance violation was re-cited in September 2025, resulting in a second $500 civil penalty. A 2022 annual inspection cited medication left unattended on the dining table. The most recent annual inspection in July 2025 cited exterior obstructions at the property; a follow-up visit in August 2025 confirmed the obstructions had been removed and noted no additional deficiencies.
