The most serious finding came in late 2025: after a resident returned from the hospital with worsening signs of infection and a need for skilled nursing services, the home did not arrange Home Health care for two weeks; inspectors concluded the home should have scheduled a doctor's appointment or transported the resident to the emergency room, and issued citations, with a fine potentially still to follow. A 2025 walkthrough found that residents in one room had no call button and no other way to signal for help, and a storage room with chemicals was left unlocked and reachable by residents. In 2024, inspectors cited the home for giving a resident the wrong medication and for staff cleaning and reusing syringes for controlled medications; separately, inspectors found two student volunteers working in the home without required background clearances, health screenings, or TB tests, and the state fined the home. Other citations over the years include reporting a resident's death 12 days after it occurred when state rules require 7, most resident rooms missing required furniture at a 2021 inspection, and a 2026 citation for residents being able to leave the home without staff knowing. One confirmed complaint found the admission agreement was missing required refund language and included contract terms not permitted under state rules.
