State inspectors have cited Sequoia Springs Senior Living Community 38 times (19 serious, 19 minor) and confirmed 26 complaints across 72 visits. Inspectors confirmed the home was understaffed on certain shifts in August 2023, including days when only one staff member was scheduled in the memory care section. That same month, a resident went 3 days without a doctor-ordered medication because the technician responsible was not fully trained and did not know where the supply was stored; inspectors also found carpets in the hallways and several rooms needed cleaning and that paint was flaking from the ceiling near the front desk. In September 2023, the home was cited for an eviction notice that was missing required legal information, including the resident's right to contest the eviction in court; the home later withdrew it. That same visit, inspectors also found the home had been using a wrong fax number to notify the Ombudsman's office about a resident's assaults, and had not notified responsible parties of each incident. In August 2025, inspectors confirmed the home refused to accept a resident back after they returned to their prior condition and were ready to leave the hospital; they were not readmitted for over a week. That same visit found the home had started a resident's antibiotic two days after a doctor ordered it to begin, and had not responded in writing to resident council concerns since January 2025. The state investigated many other complaints over these years, and most were not supported by the evidence.
