In August 2025, inspectors substantiated a finding that 2 caregivers transferred a non-verbal resident to bed without communicating their intentions, in violation of resident dignity requirements. A 2024 investigation found a resident had not received their prescribed antidepressant for approximately 5 months due to a broken medication tracking system where staff never checked refill dates. A separate 2024 complaint about a malfunctioning front-entrance security door was substantiated and resulted in a $500 civil penalty. The facility self-reported 2 resident deaths in 2025: 1 from choking during a meal in July and 1 from septic shock following a urinary tract infection in November.
