Empty medication bottles on site confirmed it. The home was then fined $800 for not submitting the required correction plan on time. Inspectors also confirmed through staff interviews that staff raised their voices at and argued with residents. A supervisor admitted he had seen it, said it was not appropriate, and gave verbal reminders, but had not written any of those conversations down. In September 2025, the state ordered the immediate removal of one staff member, though for reasons unrelated to this home.
