Since 2024, state inspectors have cited The Hills of Broadway 32 times (21 serious, 11 minor) across 26 inspections, and no complaints have been verified. During an annual inspection in August 2025, an inspector found an unlocked cabinet under the kitchen sink containing sharps and cleaning chemicals. Beginning in early 2026, inspectors made repeated unannounced visits. On three of those visits in March and April, the home had not produced documents it had agreed at a February 2026 meeting to provide. On separate visits in March, April, and May, staff members told inspectors they were receiving partial or no pay; the home was fined multiple times, including several $250 fines for repeat failures to document staff payment. On April 21, the staff member on duty said they had been paid in full, but the home had not provided the required proof of payment by the agreed deadline and was cited for that. On April 1, inspectors also found a staff member working there who was not listed as a staff member at the home and had no record of a required health screening, resulting in a $100 immediate fine; a separate $600 fine was assessed the same day for again failing to provide the agreed-upon documents. During most of these visits, the home itself was clean, residents were dressed and cared for, and utilities and food were in order.
