Since 2021, state inspectors have cited Trevista Concord 41 times (9 serious, 32 minor) and confirmed 19 complaints across 54 visits. Among the more serious recent findings: in September 2025, inspectors confirmed that after a resident with dementia wandered away and ended up at a hospital, the home told hospital staff the resident didn't want to return and refused to readmit them -- state records show that resident had dementia and was not able to leave the home unattended. That same month, inspectors found rodent droppings on a resident's dresser, floor, and bed. Also in 2025, inspectors confirmed that feces found in the outdoor courtyard was left there for over a day after staff were told about it, and that residents in memory care could not get out to the courtyard from the hallway door. In August 2025, a resident ran out of medication because the home did not order refills in time. Earlier citations include a 2022 finding that the memory care areas weren't being cleaned regularly due to a shortage of housekeeping staff, and a 2021 finding that meal utensils weren't properly cleaned. State investigators looked into many other complaints over the years, and a number were not supported by the evidence.
