Since 2021, state inspectors have cited Citrus Heights Terrace 33 times (21 serious, 12 minor) and confirmed 30 complaints across 116 inspections. The most serious incident on record: in the early hours of April 15, 2023, staff doing rounds found one resident naked in another resident's bed; the home called law enforcement, who came to investigate, and that resident was transferred to the hospital. In September 2024, a resident was found slumped in a wheelchair outside, and staff reported the resident may have been in the sun for up to three hours during a shift change; 911 was called, and the hospital treated the resident for elevated temperature and possible heat stroke. In August 2025, a resident wandered from the dining room into the hallway and fell, breaking their nose in two places and needing stitches. Inspectors looked into many other complaints over the years, and a number were investigated and not confirmed.
