In 2022, a resident was found by a family member lying on the floor after approximately 15 hours because staff failed to conduct a welfare check, mistakenly believing the resident had left for the hospital. A 2024 complaint confirmed a staff member was not assisting a resident with toileting as required by their care plan, violating the resident's personal dignity rights. Also in 2024, nail polish remover was found accessible to residents and confirmed to have been placed in a resident's mouth, resulting in a citation. A resident with documented severe dementia and a known elopement risk eloped 3 times between 2023 and 2025; in the most recent incident in March 2025, the resident was found seated in a stranger's vehicle outside the facility. In May 2025, a resident with a documented history of aggression allegedly pushed another resident, who sustained a head injury.
