In October 2025, inspectors found the home's entire emergency call-cord pager system out of service, with 2 residents reporting it had taken over an hour for staff to respond when they called for help. Inspectors also cited the home for repeated sanitary problems in one resident's room in spring 2025 and for not documenting required showers on two days in July 2024. State investigators looked into many other complaints over the years, including allegations about medication oversight, staff qualifications, and residents being left in soiled clothing, and most were not confirmed.
