Since 2022, state inspectors have cited Burlingame Senior Living 30 times (18 serious, 12 minor) and confirmed 12 complaints across 39 inspections. On October 9, 2025, the home itself reported that 2 staff members held a resident's arms down in bed while forcing the resident to take medicine. Inspectors also confirmed that a memory care resident with known wandering behaviors left a secured unit unattended and the home didn't know how the resident got out. The home's van broke down for extended periods on at least 2 occasions, residents weren't informed about other transportation options, and at least one resident had to cancel medical appointments as a result. A resident was moved into a temporary room with a broken heater that staff hadn't checked before the move, and the home shared a resident's personal information with relatives without that resident's permission. In November 2025, state inspectors delivered exclusion letters to 2 staff members, barring them from working at the home. Many other complaints were looked into over the years and were not supported by the evidence.
